Diabetic elderly people in Serra de Silves now have regular home appointments

diabetes lecture.cdrThe elderly in the parish of São Marcos da Serra who suffer from diabetes will now be monitored regularly by a team of health professionals, in their own homes.

The Diabetes in Serra de Silves project, which results from a partnership between various entities, provides for medical and nursing consultations at home, as well as health education activities for project users.

The recipients of this measure are people over 65, enrolled in the Personalized Health Care Unit (UCSP) in Silves and residents in São Marcos da Serra, who have been diagnosed with diabetes.

«After accepting to participate in this initiative, they receive the Municipality's Mobile Health Unit every fortnight, with a team composed of a doctor from UCSP de Silves and a nurse from the Community Care Unit (UCC) Rio Arade, who will then evaluate their clinical condition, preventing complications caused by this disease», according to Câmara de Silves, one of the partners in this project.

Diabetes in Serra de Silves has as its main objectives «improving the accessibility and quality of healthcare and promoting adequate surveillance among the elderly population with diabetes», with a view to the prevention and early detection of complications associated with this disease.

The project is promoted by UCC Rio Arade, in partnership with UCSP Silves, the Diabetes Functional Coordinating Unit of the Barlavento Health Centers Group, the Municipality of Silves and the Parish Council of São Marcos da Serra.
